How do you usually handle replacement batallions? Do you attach them to a brigade HQ, or just have them "loose"? Also do you attach CBs to a brigade HQ? Is that even possible considering they're brigade size themselves?

Loose.
Brigade HQs are much too expensive/valuable to waste them on replacement battalions.

Ditto on loose. I do, however, build extra capacity in my PDC to hold replacements (1 batt/brigade IIRC).
On construction brigades, my recollection is that you CAN attach them to brigade HQ (thinking of the HQ name as "HQ Brigade" might help).

While I don't have them in the actual command structure, I do notionally assign 4 replacement battalions to a division. My troop transports are built to carry a single brigade(5 battalions). So when a division is being transported the division HQ ship also has the replacements.
I tend to think and use construction brigades as mobile construction factories that don't need colonist support. Very useful for assembly of forward PDC's.

I have a similar approach, but my troop transports can carry 6 battalions, so one can carry the division HQ and the other three each have a replacement battalion.

Do they have the capacity of one factory each? That would make them a very slow and very expensive mobile construction factory. Or are they equivalent to 5 or more?

I believe one brigade has capacity of one factory. They are not very efficient but if you want to build PDC on asteroid or planet that cannot be colonized, you don't really have much choice.
