A JP can lead to a new system or an existing system. This is by design.

And yes, someone can find a new connection to your homesystem.

The exploration of a JP leads sometimes to a new Jump connexion between 2 visited sectors. 
In my current game, these (3) new connexions are "short" : never more than 2 jumps. 

Can it be longer ? Is it possible that I find a new connexion between a very far system and Sol.  And more important : is it possible that someone else find a new connexion right inside my homeworld ?
In a finite galaxy, it seems logic to find more new connexions as the exploration goes on.
